Example:

The patient is a 23-year-old female. She is a deaf student and attends NTID. Her last dental appointment was 2 years ago. Her chief complaint is pain in the left mandibular area. She states that the reason she does not seek regular dental care is that no one takes the time to explain treatment thoroughly.

Patient presents for treatment with generalized light-moderate interproximal calculus, moderate plaque & inflamed tissue. She states she is worried about her bleeding gums and she is anxious to get her teeth cleaned. (This is very basic and did not cover all of the findings)
